The following will be a break-down of the steps within a standardized treatment program so that we may better understand the positive and negative aspects that constitute the current model of addiction treatment.
Phase I. Detoxification
The initial phase of every treatment program is overseen primarily by the medical director or a physician who should specialize in addiction medicine. Unlike the following phases of treatment which are overseen by the employees at each particular program, this phase can be extremely dangerous due to the implications of physical withdrawal symptoms, and therefore needs to be under medical direction in conjunction with the treatment center. Under no circumstances should we engage in this phase without proper guidance as it can result in serious harm to the suffering addict, and even death.
Phase II. Inpatient Care
The second phase of standardized treatment is at the inpatient level of care. At this level, clients remain under 24-hour supervision and are required by law to engage in a minimum of 5 hours of programming which consists of individual and group therapy as well as case management. The extent to which any given treatment center can deliver these services in an effective manner is dependent upon the skills and knowledge of the facility. This phase last anywhere from 3-4 weeks post detox.
Phase III. Intensive Outpatient
During phase 3, the client is moved from the inpatient facility to a sober living environment where it is standard to have multiple individuals living under the same roof with one house manager overseeing their activities, keeping accountability through drug tests, and enforcing curfew. Residents of IOP programs are required to engage in 9-12 hours a week of programming. Again, the effectiveness of phase III is largely determined by the facility that is delivering services.
Phase IV. Sober Living Environment
The last phase of treatment is relegated only to the sober living environment. The logic here is that the individual has had enough programming to sustain their reintegration into society. The problem at this phase is the same as the prior two, which is that the effectiveness is largely determined not by the structure of the program, but rather by the individuals who are responsible for overseeing the clients.
In sum, standardized treatment can and has been effective for many recovering addicts. The problem, however, lies when the suffering addict would better be served in a more personalized manner.
